Scheduling an Appointment

You can schedule an appointment with us through the following methods:

  • By Phone: Call us at (734) 344-4567, Monday – Friday, 9 AM – 5 PM.​
  • Online: Complete our book an appointment request form on our website. If you are seeking a same-day or next-day appointment, our team will do our best to accommodate you if availability opens up. We ask that patients remain fully flexible, as these openings are limited.
  • We are not enrolled as a Medicaid provider and, therefore, cannot accept Medicaid plans. Patients with Medicaid coverage will be responsible for the full cost of services rendered. We recommend contacting your Medicaid plan for assistance in locating in-network providers.

Medical Appointments:

  • Full name
  • Date of birth
  • Phone number
  • Email address
  • Home address
  • Insurance information
  • Preferred pharmacy
  • Primary care physician (PCP)
  • Relevant personal health information (PHI)
  • Credit card information for co-pays and to enforce cancellation and no-show fees
  • If you are a BCN member, a referral or authorization may be required before booking. If this is not available at the time of your appointment, self-pay will be required.

Cosmetic Appointments:

  • Full name
  • Date of birth
  • Phone number
  • Email address
  • Home address
  • Relevant personal health information (PHI)
  • Credit card information to secure appointments and to enforce cancellation and no-show fees

Check-In Requirements

To ensure a smooth check-in process, please bring the following items to your appointment:

  • Photo ID: A valid government-issued photo identification.​
  • Insurance Card: Your current and valid insurance card (for medical appointments).​
  • Payment Responsibility: You are required to cover any co-pays, out-of-pocket expenses, and self-pay costs for medical appointments. All cosmetic appointments are self-pay.

Having these items prepared will help expedite your check-in process and ensure all necessary information is on file.​

No-Show & Cancellation Policy

We meticulously prepare for each appointment to ensure you receive the highest quality care. This involves allocating our staff’s time, reserving examination and treatment rooms, and organizing all necessary medical and cosmetic equipment tailored to your specific needs. By adhering to our scheduling and cancellation policies, you help us maintain this level of readiness, ensuring that all patients receive timely and attentive care.​

Surgical Procedures

  • No-Show & Late Cancellation Fee: A $150 fee will be charged for surgeries canceled or rescheduled with less than 3 business days’ notice, or missed entirely.

Medical Appointments

  • No-Show & Late Cancellation Fee: A $50 fee will be charged for appointments canceled or rescheduled with less than 2 business days’ notice, or missed entirely.

Cosmetic Appointments

  • Consultation Deposit: A $50 deposit is required to schedule a cosmetic consultation. This will be credited toward any future treatment.

  • Treatment Deposit: A $150 deposit is required to secure a cosmetic treatment appointment and will be applied to the cost of service.

  • No-Show & Late Cancellation Fee: A $100 fee will be charged for cosmetic appointments canceled or rescheduled with less than 2 business days’ notice, or missed entirely.

Injectable Appointments

  • No-Show & Late Cancellation Fee: A $150 fee will be charged for injectable appointments canceled or rescheduled with less than 2 business days’ notice, or missed entirely.

Full Payment Requirement: All cosmetic and injectable treatments must be paid in full at the time of checkout.

Business Day Clarification: If your appointment is on a Monday, you must notify us by the previous Thursday to cancel or reschedule without incurring a fee. Weekends and holidays are not considered business days.

Our Commitment to Your Time

If a provider is unexpectedly out of the office, we will notify you as soon as possible to arrange same-day care with another provider or reschedule your medical or cosmetic appointment.

This policy is effective January 1, 2025. Any policy changes made after this date will take effect immediately for all existing and upcoming appointments, regardless of when they were scheduled.
