As a parent or guardian, you have your child’s welfare foremost in your heart. We require your consent to provide necessary treatment and care to minors. This ensures your child’s welfare, your ultimate authority, and our ability to provide the level of attention we feel is warranted. Release of Medical Records, HIPAA
According to the Health Insurance Portability and Accountability Act (HIPAA) of 1996, you have a federally protected right to privacy concerning access to and third-party sharing of your medical records. To best treat you and your concerns, Ganger Dermatology will require access to previous medical documentation and your medical history.
In order to treat you safely and effectively, we require your consent to access your medical records. This form grants us permission, but you retain complete control over your medical record access for all present and future healthcare providers.

To enhance your care and stay current with evolving technologies, we may use HIPAA-compliant AI tools to transcribe our conversation to assist in documentation. Along with your medical record, transcription is securely stored for up to 7 days and is accessible only to authorized personnel. No audio recordings are saved, and you will be informed if any such technologies are being used.
